If you are applying with another person, he/she must fill out their own application form. Or download a PDF form which you can print out and mail in with your support materials. Proof of employment must be submitted before the application can be approved. Proof consists of either the three most recent pay stubs or a letter of employment verification from your present employer. In addition, a copy of the most recent W-2 form must be submitted by fax or e-mail to Fred Fragano.
